If you are a current patient at WCSC and need to refill your prescription, please fill out this form.

Please allow us 3 – 5 business days to receive and validate the information. Once it’s completed, you will receive an automatic SMS TEXT stating that your prescription has been sent to your preferred Pharmacy.

For controlled substances, keep in mind that it can take up to 7 business days to renew your prescription (since they need the providers direct approval). If you have less than 7 days please contact our Triage Nurse directly at 505-559-4495 ext. 320.
